Non-Insurance Rates

Please call us to inquire about our out of pocket rates. We accept major credit cards, debit cards, HSA and FSA cards, checks and cash as payment at time of service.

Insurance Policies

We offer insurance billing as a courtesy to our patients. We accept a variety of Insurances, Motor Vehicle Accident (MVA) and Personal Injury Protection (PIP) claims, and Triwest veterans’ insurance, along with some Worker’s Comp claims. For MVA, Worker’s Comp and Triwest claims, patients have no out of pocket cost and are covered at 100%.

We are an Out-of-network provider with the following insurance companies:
